I am not clear on the SQL Backup included in the SQL Toolbox. Does it include a server license? We purchased a SQL Toolbelt license and a server license with the understanding that the Toolbelt allowed a Backup license for another server. Is this correct?

Yes, a Toolbelt license will let you activate 1 SQL Server for use with SQL Backup.
