Placement of little comments

When maintaining stored procedures I usually put a small comment with the date on each of the lines I'm changing, and a full description of the change in a comment block at the top of the file. The little comments, such as
-- 05DEC08
are always moved to another line. I want to have them stay with the line I've altered.

Any way?

Also - how about letting me uppercase key words as a standard option? To save a keystroke.
