Removing menu items after uninstalling SQL Prompt and SQL Refactor

After SQL Prompt and SQL Refactor have been uninstalled, their menu items may still be left in SQL Server Management Studio and/or Visual Studio. When this happens, it will be necessary to remove these manually.
From the MS SQL Server Management Studio main window, click on tools, customize. Leaving this window open, click and drag sql prompt down the page onto an empty space in the left window. This should remove it from the menu bar. The procedure is similar for SQL Refactor.
The procedure for removing the menu items from Visual Studio is identical to the above.
