No SQL Prompt menu appears in Query Analyzer

It's possible for SQL Prompt tray app to be running, but no SQL Prompt menu appears in the menu bar of Query Analyzer.
First, check to make sure that SQL Prompt is configured to start with Query Analyzer, by double-clicking the SQL Prompt icon in the system tray and ticking 'Start SQL Prompt when you start Query Analyzer'
Second, there are two known issues about Query Analyzer that can mean that it is launched in a way that SQL Prompt cannot detect. This happens when
-Query Analyzer is started from inside Enterprise Manager, using the Tools menu
-Query Analyzer is started using the RunAs command in Windows.
