SQL Doc fails with 'Object reference not set to an instance of an object'

SQL Doc may fail when attempting to generate the documentation for a SQL 2000 database at the Documenting stage with the error "'Object reference not set to an instance of an object."'
There is no one single cause for this error, however, it may be a case of the SQL Server user account that was used to connect not having sufficient to all schema objects.
Using an account that has server-level administration privileges such as SYSADMIN or SERVERADMIN may resolve the problem.
